UPDATE: According to multiple sources, Perez left the game because his hamstring spasmed while he was stretching out between innings. This is peculiar, but a more encouraging first read than an arm-related concern, especially for a 23-year-old starter who missed 2024 after Tommy John surgery and remains one of the franchise’s key rotation pieces.

Eury Pérez’s afternoon at Rogers Centre ended abruptly Wednesday, cutting short one of his sharpest outings of the season and sending immediate concern through the Miami Marlins’ rotation. Pérez left after three scoreless innings against the Toronto Blue Jays, having allowed just two hits with zero walks and seven strikeouts on 55 pitches. Miami led 1-0 in the fourth inning at the time, with Otto Lopez’s first-inning RBI single scoring Xavier Edwards for the game’s only run.

While he was stretching in between innings, Eury Perez looked like he hurt himself. He immediately sat down on the Marlins bench. He needed to be helped out of the dugout and down the steps towards the clubhouse.

He had Toronto chasing through the first three innings, mixing high-end velocity with swing-and-miss finish: a 99 mph four-seamer, 96 mph sinker, 88 mph slider and 78 mph curve all showed up in the live pitch log. Vladimir Guerrero Jr. had one of Toronto’s two hits against him, but the Blue Jays had only two total bases and one runner left on through the early innings.

Pérez entered the day 3-6 with a 4.91 ERA, but he had just delivered one of his better starts of 2026 against the Mets, allowing one run over 6 1/3 innings in a 2-1 Marlins win. Wednesday looked like another stabilizing turn after Sandy Alcantara was hit for eight runs Tuesday in Toronto’s 8-1 win, which tied the series at one game apiece.

Miami’s bullpen now has to protect a tight game much earlier than planned, while the larger concern shifts to Pérez’s status. The 23-year-old right-hander missed the 2024 regular season after Tommy John surgery, returned in 2025 and remains one of the Marlins’ most important long-term arms. Any early exit from Pérez carries extra weight given that history, especially on a day when his stuff looked good enough to take over the game.
