Numerous restaurants in Forest Grove, North Plains and Banks were visited by public health inspectors in late March; some got the top grade, while others scored below 90.
Washington County’s public health department conducts unannounced, semiannual inspections of food-service establishments to assess compliance with health codes across three categories:
- Priority items: Violations that pose direct risks related to foodborne illness prevention, such as failing to discard contaminated or spoiled food.
- Priority foundation items: Issues requiring management action, equipment, or procedures that reduce potential health hazards, such as proper labeling, temperature monitoring, or record-keeping.
- Core items: Violations typically tied to general sanitation, facility upkeep, and overall maintenance.
The following Forest Grove-area restaurants received scores of 100:

- Score: 77
- Violations: A priority violation for a bourbon bottle with ants inside at Billy Scott Bar. The bottle was discarded. Another priority violation for no chlorine sanitizer in the main dishwasher in the kitchen. Chef adjusted hose and machine was tested with proper sanitizer. A violation for syrup buildup on the soda gun attracting ants, along with pink mildew in the holster and black mildew inside the ice machine. A priority foundation violation for food debris left on meat slicer and vegetable debris left on chopper in fermentation room. A priority violation for cooked mushrooms and onions stored below safe temperatures. The items were reheated. Another priority violation for ham kept past the seven-day limit. The ham was discarded. A violation for fruit stored and exposed to customers in movie theater bar. A violation for a cutting board on cook line for fish prep that needs to be resurfaced or replaced. A violation for a handwashing sink faucet spraying water in a way that would get employees wet in Bob’s Bar.

La Loteria Taco Shop, 10355 N.W. Glencoe Road, North Plains
- Score: 79
- Violations: A priority violation for improperly sanitizing and drying a knife. The person in charge moved the knife to the dishwasher. A priority foundation violation for two metal strainers with stuck-on food debris and residues hanging from the side of clean dish storage rack. A priority violation for all foods in back stand-up cooler stored at unsafe temperatures, including raw and cooked beef and chicken, cooked rice, salsa, cooked beans, coleslaw and raw bacon. A salsa bottle was also improperly stored, and raw chicken was left unattended at the prep counter. The items were discarded or moved to refrigeration. A priority violation for personal medications stored above a food preparation area next to seasonings. The person in charge moved the medications. A priority foundation violation for improperly cooling cooked meats stored in covered containers on the bottom shelf of a cooler. The person in charge removed the lids and moved items to the top shelf. A violation for no chlorine in wiping cloth sanitizer. The person in charge remade the sanitizer solution.

- Score: 85
- Violations: A priority violation for sauce bottles with visible white growths on the outside stored in the walk-in cooler next to other sauces and food containers. The person in charge discarded the bottles. Another priority violation for raw breaded chicken left out at an unsafe temperature on a prep counter. The person in charge moved the chicken to the oven for cooking. Another priority violation for open containers of salsa and cream cheese kept past the seven-day limit in the walk-in cooler. The person in charge discarded the products.
Rogue Brew Pub and Eatery, 31500 N.W. Commercial St., North Plains
- Score: 86
- Violations: A priority violation for cooked brisket left out on the counter at improper temperature. The brisket was discarded. A priority foundation violation for sliced meats, chili and marinara sauce not properly date-marked, a repeat violation. The person in charge relabeled the items with proper dates and discarded the marinara sauce. A priority foundation violation for fries left out without a discard time marked or proper tracking for when they needed to be thrown away. The person in charge marked the fries to be discarded within four hours.
